Crew duty time, including any required overnight if the mission stretches beyond a single duty period. Clinical team configuration, physician plus nurse baseline, with specialist crews (neonatal, paediatric, ECMO, bariatric) costing more. Equipment configuration, ICU baseline with specialist additions per mission. Permits and overflight clearances in every jurisdiction on the route. Ground ambulance at both ends.
Add receiving hospital coordination, customs, immigration, and any documentation overhead, and that is the build. There is no flat rate because there is no standard mission.

Short regional medevac under 1,000 nautical miles, light or mid-size jet, ICU baseline configuration: typically AUD $25,000-$80,000.
Medium-haul international medevac of 1,000 to 4,000 nautical miles on a mid-size or long-range jet: typically AUD $80,000-$250,000.

Specialist missions, ECMO, neonatal incubator, bariatric, multi-patient or infectious disease isolation, add a defined increment over the baseline mission, depending on configuration and the additional crew.

An air ambulance is a dedicated aircraft, a specialist clinical team, ICU-grade equipment, full international permits, and ground coordination, committed to one patient for the duration of the mission. Operators that quote substantially lower numbers are usually missing something, clinical capability, equipment configuration, insurance status, contingency planning, or the discipline to walk away from a mission they are not equipped to run. Cheap medevac is rarely actually cheap by the time the mission completes.
Insurance and assistance coverage
Most international travel insurance policies include medevac cover, typically coordinated through an assistance company. Corporate, NGO and government health policies often include cover with broader scope. The patient and family rarely transact with the medevac operator directly when assistance cover is in place, case authorisation, costing, documentation and payment are routed through the assistance company.
