Medical Escort Flights: Seated Escort on Commercial Aircraft

Whether a patient can travel with a seated medical escort, rather than requiring a stretcher or dedicated air ambulance, is a clinical decision made by the treating team and, for commercial bookings, confirmed by the airline's own medical clearance process. A medical escort flight is a scheduled commercial service on which a patient travels in a standard passenger seat, accompanied by a qualified nurse or doctor whose role is to monitor the patient's condition, manage any medication or basic clinical needs in flight, and coordinate with ground medical services at departure and arrival. It differs from a stretcher booking in that the patient does not require a lie-flat position, and from a dedicated air ambulance in that no chartered aircraft or specialist cabin equipment is involved.

Seated medical escort travel is generally considered when a patient is stable enough to sit upright for the duration of the flight, does not require continuous monitoring equipment beyond what can reasonably be carried on board, and has a condition where the primary risk is not the flight itself but the practicalities of self-managing travel, mobility limitations, cognitive impairment, post-procedure vulnerability or a condition requiring medication administration en route.

• Patients recovering from a procedure who need supervision but not lie-flat positioning
• Patients with reduced mobility, cognitive impairment or anxiety requiring hands-on support during travel
• Patients requiring scheduled medication, oxygen at prescribed low-flow rates (where the airline permits), or routine observation in flight
• Repatriation cases where the clinical picture has improved to the point a stretcher or dedicated aircraft is no longer required

Seated escort travel is not appropriate where a patient's condition requires lie-flat positioning, continuous intensive monitoring, ventilation, infusion therapy that cannot be managed in a standard cabin, or where clinical risk during a diversion or delay on a scheduled service would be unacceptable. Two separate approvals sit alongside each other on every medical escort booking: the clinical determination of what level of escort the patient needs, made by the treating team, and the airline's own medical clearance, which independently assesses fitness to fly for that specific service.
