Medical Repatriation Cost: What Drives a Repatriation Flight Price

Medical repatriation, flying a patient home for ongoing treatment or recovery, is priced bottom-up against the actual mission. There is no published rate card because there is no standard repatriation: the price is determined by the aircraft type, total flight hours including positioning, the clinical team configuration, the equipment carried, the bed-to-bed ground links at both ends, permits, fuel and any specialist clinical requirements.

Operators that quote a flat repatriation rate without knowing the patient profile and the route are either over-pricing to cover risk or under-pricing on assumptions that get revised after commitment. Aviall returns a written, itemised quote with the cost drivers visible so the patient, the assistance company and the family can see exactly what is being paid for and why.

A handful of factors drive virtually all medical repatriation pricing:

• Aircraft type, light jet, mid-size, long-range or ultra-long-range; larger aircraft cost more per hour but cover the route in fewer hours
• Total flight hours including positioning legs (the empty flights to bring the aircraft to the patient)
• Fuel, the largest variable cost in any charter, sensitive to route and tankerage
• Clinical team, physician and nurse baseline, with specialist crews (neonatal, paediatric, ECMO, bariatric) costing more
• Equipment configuration, ICU baseline, with specialist additions added per mission
• Permits and overflight clearances on the route, which vary substantially by jurisdiction
• Bed-to-bed ground ambulance links at both ends, including any specialist transport
• Receiving hospital coordination, customs, immigration and documentation

Short regional repatriation (under 1,000 nautical miles, light or mid-size jet, ICU baseline): typically AUD $25,000 to $80,000.

Medium-haul international repatriation (1,000-4,000 nautical miles, mid-size or long-range jet): typically AUD $80,000 to $250,000.

Long-haul intercontinental repatriation (4,000+ nautical miles, long-range or ultra-long-range jet, possibly with one tech stop): typically AUD $200,000 to $600,000.

Specialist missions (ECMO, neonatal, bariatric, multiple patients) add a defined increment depending on configuration and crew, quoted transparently against the baseline mission.

Most international travel insurance policies include repatriation and medevac cover, typically coordinated through an assistance company. Corporate, NGO and government policies often include cover with broader scope. Aviall liaises directly with assistance companies and insurers on case authorisation, costing, documentation and payment, the patient and family rarely transact with Aviall directly when assistance cover is in place.

Where there is no insurance cover, payment is typically pre-flight by bank transfer. We work with families and corporates on payment timing where the situation requires it; the priority is the clinical mission, not the paperwork.

A repatriation quote that can be relied upon, and that will not change after commitment, should include all of the following in writing:

• Aircraft type and operator
• Routing including any tech stops, with reason
• Realistic activation window from confirmed booking
• Clinical team composition and qualifications
• Equipment configuration including any specialist devices
• Bed-to-bed ground links at both ends
• Permit status on each leg
• All-in price with no hidden positioning, fuel, handling, permit or crew-duty extras
• Payment terms and assistance / insurance liaison status
